FINE PRINT & TERMS OF MEMBERSHIP

• Our Memberships have a minimum three (3) month commitment. There is no freeze or pause.
• After the minimum commitment is fulfilled, a cancellation request can be submitted in writing a minimum of 30-days prior to your bill date.

• Payment is taken when you sign up for the membership. For subsequent months, you will be billed on the same day of the month as your sign-up date.
• Should payments be declined or missed for more than one (1) month, the membership may be subject to cancellation.
• Remember: Use it or lose it! Membership treatments must be redeemed by the member each month and cannot be shared or transferred to another individual who has not purchased this program.
